中醫舌診標準化之研究(3-3)-特定疾病舌苔證型資料庫之知識探勘特徵 邱創乾逢甲大學 |

| 、聞、問、切四診是中醫辨証的依據。望診是觀察人體的外形、氣色及舌象，而舌象的觀察是望診中非常重要的部份。舌診主要是觀察舌質和舌苔，舌質亦稱舌體，由舌的肌肉脈絡組成，舌苔為黏著在舌體上的苔狀物質。舌質一般是觀察舌頭的形體大小，顏色變化，運動是否異常。舌苔的觀察則以苔色的變化、分佈情況以及濕潤程度為主。數千年來中醫師憑著這些舌診特徵參數來作為臨床診斷的依據，其診斷的正確率往往取決於醫師的主觀經驗及當時所在環境的各種因素，診斷結果常因人而異又無法達到研究或臨床上重覆性的要求。近二十年來，中醫開始朝科學化邁進，藉由客觀的實驗及數據來證明中醫的理論。在中醫舌診的發展上，利用電腦化舌象診察系統來對舌象特徵作定性與定量的分析是必然的趨勢。 過去幾年國內舌診相關研究發展了幾套舌象診察系統，由於各系統對於舌診特徵參數定義不一致，以至於量化分析結果會有所差異，也因此不同系統對於相同的舌象特徵會有不一致的量化結果，所以舌診標準化確實有高度的研究價值。本計畫是整體中醫舌診標準化研究的一項子計畫，第一年我們匯集舌診專家的意見，訂定出具有臨床應用價值的舌象特徵參數，同時制定舌象判讀及量化標準程序，並且依照這些程序對一百例常人舌象作定性與定量的分析。在第二年以色彩對應表的方式來建立舌象影像色彩相容轉換機制，藉以確保舌象影像判讀及量化的品質。另外以彩色紋理的舌象紋理偵測方式來提升舌苔紋理的識別率，最後對特定疾病患者進行舌苔的定性與定量分析與其他相關生化檢查數據的收集工作。在先前研究的基礎上，本年度的計畫是持續收集特定疾病的舌苔證型資料庫，並結合統計及醫學專家，來進行知識探勘的研究，並與其他子計畫整合，共同歸納出具有臨床應用價值的知識，以中醫界教學、臨床、研究及衛教推廣使用，使得中醫舌診可達到標準化、國際化的目的。 | |

| The principles of traditional Chinese medical diagnosis are based on the information obtained from four diagnostic processes, which are inspection, listening and smelling, inquiry and palpation. Among these diagnostic processes are mentioned above, the examination of tongue is attached great importance for getting significant evidences in diagnosing the patients health condition. Generally, tongue diagnosis can be obtained by means of observing the substance and coating of the tongue. The substance of tongue, can be characterized by its color, luster, shape and movement, consists of muscle, blood vessels, nerves and mushroom nipples. The coating of tongue that adhered to substance of tongue consists of food dregs, germs, and saliva. It can be distinguished by its color and certain textural properties.
