高、低頻率電針手、足三里穴對人體胃電圖和胃腸道激素之影響

中國醫學使用針灸治療胃腸疾病已有數千年的臨床經驗。而在臨床上，足三里穴仍是最重要及最常被運用的穴位之一。古人云：“肚腹三里留”，其實可包含足三里和手三里兩穴。因足三里和手三里分別屬於陽明經胃及大腸的穴位，兩者皆有治療胃腸疾病的療效，故用兩穴來研究和比較針灸對胃腸生理之影響，較能反應其穴位特異性。
胃腸的運動及分泌功能受自主神經和內分泌的控制與調節。胃電圖是一種方便且無侵害性的儀器，它常被用來記錄胃的肌電活動情形，可做為胃功能活動的客觀生物電學指標。此外，胃腸道激素如胃泌素、胃動素、胰多胜、胰泌素、膽囊收縮素…等等，也會影響胃腸運動及分泌的作用。因此，胃電圖和胃腸道激素兩者皆可用來探討胃腸生理。
先前的研究指出，低頻電針刺激可以引起腦啡的釋放，而更高頻電針刺激導致強啡的釋放。因此，我們知道在不同頻率電針刺激下，可得到不同的生理反應。據研究在胃腸道也存在著類鴉片物質受體。所以針灸所釋放的內源性嗎啡物質應當也會作用於消化道，產生生理上的變化。到目前所知，高頻和低頻電針刺激對於人體胃腸功能的影響為何，尚未被研究清楚。
因此，為了瞭解針灸與胃腸生理的相關性，本研究選擇十六名健康年輕男性為實驗對象，分別接受以下五組實驗：1)2Hz電針足三里組；2)2Hz電針手三里組；3)100Hz電針足三里組；4)100Hz電針手三里組；5)非穴位針刺對照組。每組實驗分為針刺前期、針刺期、針刺後期，每期時間為二十分鐘，實驗時間總共為六十分鐘。我們觀察了在不同的時期，胃電圖和胃腸道激素的變化情形，希望經由實驗結果來比較高、低不同頻率電針刺激的異同點，及穴位的特異性，以便進一步探討針刺對胃腸道運動的作用機理。實驗結果以重複測量變異數分析（ANOVA）做統計分析處理。

The effects of electroacupuncture of high and low frequencies at Shousanli and Zusanli acupoints on electrogastrography and gastrointestinal hormones in humans.

Acupuncture has been used empirically to treat gastrointestinal disorders in traditional Chinese medicine for several thousand years. Zusanli(ST36) and Shousanli(LI10) points belong to the stomach and large intestine meridians, respectfully. They all have the effects in treating gastrointestinal disorders. It could search of the effect of acupuncture on gastrointestinal physiology between the two acupoints, because they could reflect the specificity of acupoints.
Gastrointestinal motility and secretion are controlled and regulated by autonomic and endocrine system. Electrogastrography(EGG) is a noninvasive tool for the measurement of gastric myoelectrical activity. In addition, gut hormones ex. gastrin, motilin, pancreatic polypeptide, CCK…etc., also can influence the gastrointestinal motor and secretion. So we do this study with EGG, gut hormones as the detector for gastrointestinal function.
