建立自動化中草藥不良事件通報系統之研究

本研究的主要目標是建立一套架構於中醫診間醫令系統上的中草藥不良事件自動通報資訊系統，這套系統將提供並預計可整合於各醫院的中醫醫令系統中，預留透過資訊科技自動集中通報之功能彈性，由下而上達到有效整合中草藥不良事件通報的效果。根據文獻，每年美國約有九萬八千人因醫療疏失而導致死亡，為美國第八大死因。經統計約為千分之三的住院病患因ADEs而死亡，其中28%~56%是可預防的。傳統觀念認為純中草藥或相關製劑無副作用，近幾年來透過相關資料之分析報導顯示這是不客觀的。根據中國大陸的研究，50年代及其以前，中藥不良事件只有26例，60年代147例，70年代398例，80年代則高達2217例。再加上其他國家陸續傳出的報告指出使用中藥混合方劑治療皮膚濕疹造成病人持續出現噁心、意識不安、體重下降，甚至有黃疸之肝炎現象。以及因服用中藥減肥因服用中藥減肥，造成100多位病人急速腎功能惡化終身洗腎，證明無論誇大或隱瞞中藥不良反應的誤導宣傳，對於中醫的研究與推廣均有害無益。因此建立中藥安全性或中藥不良反應的系統性研究是目前當務之急。本研究將以在中醫醫令系統上建構中醫藥不良事件通報介面的方式，減少因通報資訊非匿名、不良反應結果選項過少以及通報表填寫複雜費時等疑慮，並配合張恆鴻副院長『中藥不良反應通報中心』的郵遞通報方式，在各級醫院、藥房等建立配合中醫醫令系統的不良事件通報功能，透過各單位整合統計後自動通報，提高中醫藥不良事件通報效率。本研究預計將標準化中醫藥不良事件通報資訊，有系統的整理國內外中醫藥不良事件資料庫，計畫相關人員將可得到中醫藥不良事件之觀念、對於中醫藥不良事件與中醫醫令系統的整合觀念，並對於未來電腦在中醫藥處理上的應用作一有效的貢獻。

Implementation of an automatic reporting system for Adverse Traditional Medicine Event

The purpose of this research is to implement an automatic reporting system for adverse traditional medicine events (ATMEs). The system can efficiently increase the rate of ATMEs reporting by being integrated into traditional physician order entry system and report automatically to central server with further interface definition. According to the literature review, preventable adverse events are a leading cause of death in the United States. When extrapolated to the over 33.6 million admissions to U.S. hospitals in 1997, the results of these two studies imply that at least 44,000 and perhaps as many as 98,000 Americans die in hospitals each year as a result of medical errors. Traditionally people in Taiwan believe that Chinese herbal medicine are harmless, however, the report from Mainland China disapproved this wrong impression.
