過敏性鼻炎針刺治療前後之微陣列分析

基因微陣列 (Microarray) 為近年來新發展的分類與鑑定的工具，本計畫微陣列實驗由長庚醫院及長庚大學的「基因體醫學研究核心實驗室」(Genomic Medicine Research Core Laboratory-GMRCL)，進行「過敏性鼻炎針刺治療前後之微陣列分析」，利用血液檢體進行cDNA microarray，以測量針刺治療前後之免疫反應基因與過敏性鼻炎臨床資料的相關性。 本計劃將探討針刺對過敏性鼻炎的分子生物學與免疫學上的機轉，患者進行針刺治療，在治療過程中，抽取病人血液檢體為材料，進行microarray以測量免疫反應基因的變化。

患有過敏性鼻炎的病患常會合併其他的過敏性疾病，如異位性皮膚炎、過敏性結膜炎、蕁麻疹以及過敏性氣喘。 而近年的WHO報告ARIA (allergic rhinitis and its impact on asthma) 指出，氣喘和鼻炎常發生在同一個病人身上，所以提出「單一呼吸道，同一疾病」的觀念，這些論述與傳統中醫學的觀點『肺主皮毛』有著相近的說法。

臨床上許多過敏性鼻炎患者，由於無法獲得滿意的療效，而求助於傳統中醫藥以及針刺的幫助，針刺具有良好的治療與保健作用，從免疫的角度對針刺作用探討研究顯示，針刺可以調整體內各種不同的免疫物質趨于穩定和平衡，使之有利于人體生理機能，近年已有研究指出，以針刺治療氣喘或過敏性鼻炎，可以有效改善免疫反應。 本計畫希望藉由結合中醫部門臨床醫師與GMRCL實驗室的豐富Microarray經驗，更深入探討針刺對過敏性鼻炎的作用機轉。 以基因導向醫學觀點檢驗傳統中醫學常見之「同病異治」與「異病同治」。

預計針對18-45歲出現打噴嚏、流鼻水及鼻塞等三大症狀的患者，且治療前一個月未曾服用中藥或西藥者，給予針刺治療。 病患將接受為期4週、每週3次，總計共12次的的針刺治療。 並在針刺前、針刺後第10天、針刺後第20天與接受全部針刺療程後，對病患抽血 (總計進行4次，每次抽血5 ml)，進行周邊血液分析以及Microarray分析。 Microarray結果分析將研究：(I) 過敏性鼻炎患者針刺治療前後，血液中基因表現的改變。 (II)血液中基因表現的改變與特定血球數量的關連性。 最後將會針對病患在治療前、中、後之臨床症狀做統計分析，並與Microarray結果分析做關聯對照，以獲得最後之結論。

The cDNA Microarray analysis for allergic rhinitis treatment in Traditional Chinese Medicine

This project「The cDNA Microarray analysis for allergic rhinitis treatment in Traditional Chinese Medicine」 will be performed at Genomic Medicine Research Core Laboratory-GMRCL in Chung Gang University and Chung Gang Memoral Hospital. Allergic rhinitis patient will receive acupuncture treatment, blood sample was attained before and after the acupuncture therapy. Then we use cDNA microarray to analyze the immunomodulatory effects of acupuncture in the treatment of allergic rhinitis.

Allergic rhinitis patient were usually accompanied with other allergic diseases, such as atopic dermatitis, urticaria and allergic asthma。 According to the reports of WHO ARIA (allergic rhinitis and its impact on asthma)，asthma and allergic rhinitis often happened in the same person, and the theory ”one air way，one disease” was pointed out, there was a similar theory in the traditional Chinese medicine ”the lung controlling skin and hair”.
