白芍鎮痛機轉之研究—其主成分Paeoniflorin與NMDA接受體間關係之研究

白芍為臨床上常用之婦科要藥,具有調經止痛作用,常見方劑如四物湯等。由我們先前之研究結果發現,白芍具有鎮痛、鎮靜、抗痙攣及降溫等作用;進一步分離其主成分Paeoniflorin,發現亦具有鎮痛作用,然而目前少見有關探討其作用機轉之研究報告。從本研究室之預試驗中發現,Paeoniflorin可加強嗎啡之鎮痛作用及減弱注射高劑量嗎啡所引起之類似痙攣作用,而後者被認為與中樞NMDA接受體之興奮有關;雖然嗎啡在臨床上常用於嚴重疼痛之減緩,然其易形成耐受性及依賴性,則是嗎啡使用上最困擾之處。慢性投予嗎啡所造成之依賴性與耐受性亦被認為與NMDA接受體之改變有關,因此本研究之主要目的在探討Paeoniflorin與NMDA接受體間之關係,將來期望能進一步瞭解Paeoni-florin是否能改善嗎啡所造成之依賴性與耐受性,以提供白芍在臨床應用之參考。

Studies of analgesic action mechanisms of Paeonia lactifloria PALL.—The relationship between paeoniflorin and NMDA receptor.

Paeony roots (Paeonia lactifloria PALL.) has been used in gynecological disorders as an antispasmodic and analgesic drug. From our previous studies, we found that Paeonia lactifloria has analgesic,sedative, anticonvulsive and hypothermic effects. Its active component,paeoniflorin, was also found to have analgesic effect. However, there’s only few papers mentioned about the analgesic mechanism of paeony roots. Morphine is used to relieve cancer pain. However, tolerance and dependence caused by morphine usage which often bother patients. From our screening studies, we found that the major component of paeony roots, paeoniflorin, can potenitate morphine-induced analgesia. Paeoniflorin can attentuate the seizure-like exictation induced by high dose morphine, which is considered to have a close relationship to NMDA receptors.
