中醫方劑「甘露飲」配合放射線治療鼻咽癌引起口乾和粘膜發炎(Mucositis)治療之研究

「鼻咽」是現代醫學的解剖名詞。古時候《內經》提出的「頏顙」一詞，相當於現代醫學的鼻咽部。中醫傳統醫學認為「鼻咽癌」的病位在頏顙，屬「失榮」範疇(1)。因為本病後期，面容憔悴，形體消瘦，狀如數木失去榮華活力，枝枯皮焦，故名「失榮」(2)。

鼻咽癌好發於45~55歲得男性，為台灣最常見之頭頸部腫瘤。根據1991年衛生署的報告，它是十大癌症的第八位，是癌症死因的第七位。每年有一千多名病患，給病家庭極大的經濟衝擊而造成社會很大的問題，因此如何能對鼻咽癌有更好的控制率是台灣醫療上重要之課題(3)。放射線治療是公認鼻咽癌治療的首選方法，早期治療的控制率可達到80~90%，晚期腫瘤的控制率卻只有40~50%而已。分次的放射治療，雖可增加15~20%的腫瘤控制率，病人卻得付出較嚴重急性輻射傷害的代價(4)。

Mucositis粘膜發炎是鼻咽癌放射線治療後常見的併發症。文獻顯示有1/3鼻咽癌的患者在接受放射線治療一週後，口腔黏膜有紅(erythema)、腫(edema)、痛(pain)的症狀出現，放射線治療3週後，口腔黏膜損傷甚至潰瘍(ulcer)。影響患者平日的進食吞嚥、說話及睡眠之品質(5) (10) (11) (12) (13)，致使有些患者放棄放射線治療，殊為可惜。或是病人雖勉強接受放射線治療的計劃，卻不免影響其生活品質。

由於甘露飲劑尚未有防治鼻咽癌患者口腔黏膜Mucositis的療效報告，目前長庚醫院中醫部延續與放射腫瘤科進行「中醫脈診儀應用於鼻咽癌患者接受放射治療全程之研究」(CCMP87-RD-038)，擬繼續合作進行「中醫藥配合放射治療對鼻咽癌患者療效之相關研究」。本研究計劃收集60個可完整追蹤評估的鼻咽癌患者，隨機分為中醫方劑(甘露飲)加放射線治療之病例組(30位)和單純放射線治療之對照組(30位)，以進行兩者口腔黏膜炎Mucositis療效之觀察及比較。

Mucositis and dry mouth to NPC radiotheraoy treated by Chinese herb Gan-Lu-Yin

Nasopharyngeal carcinoma(NPC) is one of the ten leading cancer in Taiwan, and usually occurs in the middle age of male (45~55 y/o). Therefore, the better contrl of this tumor becomes one of the major medical issues. Currently, radiotherapy is the best method to treat NPC. Although a fair result could be expected for the treatment of the early stage of NPC, most of the treatments for late stages of patients still are low in control rate. In additions, radioherapy may induce some acute or late complications such as mucositis or dry mouth which may bother patients and further interrupt the course of treatment. Unfortunately, there is no better treatment to relieve the complications of radiotherapy.

In the present study, we plan to treat mucositis and dry mouth complications due to NPC radiotherapy by Chinese herb medication (Gan-Lu-Yi) 30 NPC cases only accepted radiotherapy and the other 30 NPC cases treated by radiotherapy with Chinese herb (Gan-Lu-Yi). From this study, w
