腐盡生肌散促進進傷口癒合作用機轉之研究

腐盡生肌散為中醫藥古籍中記載可促進傷口癒合之良方，以往吾人曾以腐盡生肌應用於臨床上壓創( pressure sore )之治療，獲得顯著之療效(DOH 84-CM-058；然而究竟此類方劑促進傷口癒合之作用機轉為何則仍屬未知且缺少相關之基礎研究.傷口癒合(Wound healing )為一複雜之生理作用，其中上皮再生(epithelialization)的過程中極為重要的步驟為傷口邊緣(wound edge)的表皮細胞(epithelial cell)的增生(proliferation)，而刺激表皮細胞增生的重要因素為傷口局部區域角質細胞(keratinocyte)所釋放出的生長因子(growth factor)，這些生長因子包含了 Transforming growth factor-a (TGF-β )，Transforming growth factor-b (TGF-β )，Plateletderivedgrowthfactor(PDGF)，Epidermalgrowthfactor(EGF)，Fibroblast growth factor(FGF)等，以 autocrine 或是juxtacrine 的方式作用於分泌角質細胞本身或是或是鄰近之角質細胞；由於在傷口癒合過程中表皮細胞之行為與表現與培養角質細胞(cultured keratinocyte)極為類似，因此本研究擬以中藥方劑腐盡生肌散體外實驗模式(in vitro)作用於培養角質細胞(cultured keratinocytes)，並以ELISA法測定受作用後的培養角質細胞其生長因子(growth factors)分泌量之變化，期望能因此了解中醫藥促進傷口癒合之生理機轉，進而達成中醫藥科學化的理想。

The study of the mechanism of the effect of the Fu-Jin-Sheng-Jih-Sun on promoting wound healing

Chinese herb medicine had long been used to promote wound healing with obvious effect,but the underlying mechanisms are still not clear.Wound healing involves complicated processes including reepithelialization. Repithelialization in the wound edge takes place with epithelial cells proliferation and migration.The keratinocytes in the wound edge can secrete growth factors (TGF-β ,TGF-β ,PDGF,EGF,FGF) to promote wound healing through autocrine and juxtacrine action on the secreting keratinocys and neighboring keratinocytes.In this study, Chinese herb medicine(Fu-Jin-Sheng-Jih Sun) will be incubated with cultured keratinocytes and the secretion of growth factors from keratinocytes will be quantified before and after the incubation with ELISA.It is expected that the underlying mechanism of the effects of this Chinese herb medicine on wound healing can be partially explained by the results of this study.
