中醫基層醫療院所參與臨床研究模式之建立與影響評估─以中醫腦血管疾病後遺症門診照護計畫為例

台灣中醫相關研究所已取得碩士學位者超過600人，取得博士學位者將近100人，惟國內中醫師研究人才多數從事於臨床醫療服務，其中將近90%的中醫師於基層院所執業，較少有機會參與中醫藥研究發展，中醫師研究人才的流失，無疑將使中醫藥的研究規模受到限制，研擬鼓勵中醫藥人才從事中醫藥研究之具體措施，使其發揮所長，促進中醫藥之發展，為當今中醫政策研究的重要課題。
本研究主要以(1)問卷調查及資料蒐集方式進行中醫師研究人力現況調查與分析並進一步探討中醫師研究人力繼續從事中醫藥相關研究的影響因素，(2)利用全民健保承保抽樣檔2005-2009年、腦血管疾病病人西醫住院中醫會診計畫2006-2010年、及腦血管疾病後遺症門診照護計畫2010年中醫醫療費用申報資料探討台灣腦血管疾病病人於中醫醫院、西醫醫院附設中醫部門與中醫基層診所的醫療利用情形，並比較其差異，(3) 召開專家學者會議方式研訂中醫醫療院所參與腦血管疾病後遺症照護計畫臨床研究之組織架構、運作模式及推展方式。
本研究係以中醫腦血管疾病後遺症門診照護計畫為例 ，建構中醫基層醫療院所中醫師參與臨床研究之模式，並對國內中醫藥研究發展可能產生之影響進行評估。研究成果將可提供衛生主管機關訂定中醫藥衛生政策之參考，進一步提昇中醫醫療與照護品質。

The establishment of model for assessing the effectiveness of clinical research in traditional Chinese medicine clinics: A TCM care plan for stroke

More than 600 persons had obtained master degree of traditional Chinese medicine (TCM) and there were approximately 100 persons were doctors of TCM in Taiwan until the present time. However, most TCM physicians had low opportunity to participate medical research because 90% of them practice in primary clinics. The loss of manpower of TCM physicians in medical research may limit TCM research. To encourage TCM physicians participating TCM research and to promote the development of TCM research is crucial for the policy of TCM research.
In this study, we will first conduct questionnaire-based survey to report the patterns of TCM physicians participating TCM research and associated factors. Second, we will use national health insurance research database (2005-2009), data of hospitalized stroke patients consulting TCM (2006-2010), and data of outpatient care for the stroke sequela plan (2010) to analyze the utilization of TCM use among stroke patients in primary TCM clinics, TCM outpatient service in hospital, and TCM inpatient service. Third, we will hold a conference and invite experts to draw up the scheme, model, and procedure for TCM primary clinics to participate clinical research in caring stroke sequela.
This study will focus on the plan of TCM outpatient care for stroke sequela and try to establish a model for TCM primary clinics participating clinical research. Another purpose of this study is to asses the influence on TCM research and development in Taiwan. The result of this study may provide reference for health authority to set up TCM health policy and promote health care quality in TCM services.
