過敏性鼻炎患者中醫體質類型與基因及蛋白質表現之相關性研究

中醫的診斷利用人體在疾病的表現狀況，將人的體質分成寒、熱；虛、實；表、裡等等類別，利用屬性不同之中藥將不平衡的身體調至平衡以治療疾病。數千年來，體質特性與相關疾病間的關係及其治療方法均已詳細的紀錄在中醫的典籍裡，不同中藥的藥性與其對人體的影響也有詳細記載。近幾年，人類基因序列已公布，許多分子生物的技術與資訊提供很好的方法來分析細胞的整體表現，突破以往以一個基因為主的研究方式。這種以大量基因表現分佈的研究方式與中醫的診斷方式，利用個體的特徵來診斷疾病有相似之處。
在92年的計畫中，我們採用臨床試驗模式，收集約60名經西醫確診為過敏性鼻炎患者，隨機分為二組分別服用寒或熱性中藥方二個月，在服藥前後分別接受中醫體質分型及西醫臨床及實驗室評估，並以二維電泳膠（2D-gel）蛋白質分析的技術分析病患血清中蛋白質的變化，以研究中醫寒或熱性體質患者的蛋白質表現，及服用不同屬性寒熱中藥之影響，進而探討蛋白質表現之相關基因，作為尋找與體質「寒熱」類型及過敏性鼻炎有關係的基因之依據；其中我們發現有三個蛋白質的差異與服用寒與熱藥物有正相關。延續92年的計畫，預計完成以下目標： 1. 將2D-gel所確定的三個蛋白質再做定序，以確定所代表的基因並利用Western blot做確認，並尋找該基因與寒熱藥物處理的機轉。2. 在台中榮民總醫院與中國醫藥大學重新收集100名病患，利用不同臨床中心的病患做比對，使所收集的資料有較高的可信度。3. 比對並找出寒熱不同中藥方對不同體質患者基因及相關蛋白質表現之影響。4. 比對並找出寒熱不同體質患者之基因及相關蛋白質表現。
此研究所確認的基因將可以用來研究寒、熱藥物與寒、熱體質的分子生物機轉與調控、基因的差異性與疾病的關係，進而做為其他中西醫結合診療研究之基礎。所建立之研究方法將提供一個嶄新的領域來研究中醫理論的科學性，其結果更可以用來建立中西醫之間的橋樑、並分析個體之間的基因差異，希望數千年中醫的人體試驗結果能利用現代分子生物的方法找出身體各症狀與分子生物特性的相關性，並進而促進醫學上的進步。

The Study of the Relationship between Gene & Protein Expression and the Chinese Medical Constitutional Types in the Patients of Allergic Rhinitis

In Chinese medicine, the patients’ constitutional types are classified according to their manifestation in disease circumstances, and different types of Chinese herbs are used to treat the patients by correcting the unbalanced status of body. In recent years, instead of the method of studying single gene, many molecular biological information and techniques provide very good methods to analyze the whole expression of the cell. The method to investigate the expression of large quantities of gene and the Chinese medical diagnostic method have similarity that they make use of the characteristics of individual for diagnosis.
In order to find out the related genes for the “cold” and “hot” constitutional types in the future, last year, we started with the 2D-gel protein analytical methods to study the protein expression of the two groups of patients, which belongs the “hot” and “cold” constitutional types respectively. And we also study the effects of the “cold” and “hot” types
