中草藥副作用主動通報監視系統的建立與研究

研究目標為協助衛生署中醫藥委員會，逐步建立國內中草藥更安全的使用環境，保障國民健康，形成一個有關中藥安全資料的技術平台，讓中藥的安全性，不再停留於古籍的記載上，而有更好的實證醫學證據。對於國內中草藥健康產業的發展與外銷推展，將形成一個「中草藥安全性」把關的重要基石，此一行銷保證，更能促使國內中草藥健康產業進軍國際。首先對國內外文獻與國內現有的通報個案，進行因果關係的初步判定，並逐步建立適當的判定基準。其次，利用健保資料的與癌症登記資料、肝腎之重大傷病登記、及死因資料之連結來進行流行病學分析，計算過去在醫師處方模式下的中藥方劑使用的狀況，找出使用頻率最高的方劑，以及是否造成罹癌、洗腎、肝硬化、肝癌或對人體有無產生各種死因發生之增加。並針對使用過中藥的健保資料中，找出罹患B、C型肝炎患者，比較與未使用含肝毒性中藥族群罹患肝癌肝硬化的勝算比有無差異。同時第二年起將對於用量廣泛、文獻及個案報告懷疑有可能造成肝、腎或血液傷害之方劑，將聯合高雄市立中醫醫院、中國醫藥大學附設醫院中醫部、長庚中醫院、台北市立中醫院、陽明醫院中醫部及台大中藥臨床試驗中心研究人員及中醫師們共同判定方劑優先次序，發展出一個中藥安全性主動監測系統，對1000位病人作定期副作用監測及血、尿液用藥前後之檢驗；第三年則擴充到1200人。此系統將逐漸擴及更多中醫師及藥師，以便逐步與目前長庚中醫院所建立之中草藥不良反應通報系統結合，來對現行中草藥用藥的安全性進行監測，以去蕪存菁。第一年除了實際進行各種研究外，也將在年度末邀請中醫藥專家共同研擬中藥安全研究發展策略，以協助鈞署中藥用藥安全環境之建構。

The establishment and rearch of an active surveillance system for the use of herbal medicine

It is expected that the establishment of CM safety database and the related techniques will make CM use an evidence-based clinical practice. It does not only contribute to the development of health industry in Taiwan, but also form an important safeguard for bringing and marketing CM products in the international markets.
