CCMP102-RD-009 中醫藥治療子宮內膜異位症婦女之療效評估及對助孕酮受器表現之影響

研究目的:目前台北市立聯合醫院之陽明院區設有中西醫整合中心專門服務照顧解決不孕症夫妻之不孕問題,經本院醫師團隊整合的治療經驗發現在婦女方面引起不孕症的最棘手為首的是子宮內膜異位引起的不孕症。目前台灣地區有關於子宮內膜異位症的臨床研究並不多,致使許多婦女轉向本團隊整合門診之中醫求助。中醫依據患者的主訴以辨證分類進行治療,在治療婦科疾病上已累積並記載多年的人體用藥經驗。因此本研究目的在評估中藥臨床療效及安全性,以中醫辨證論治療治療子宮內膜異位症為主題,針對改善婦女子宮內膜異位症狀與受孕之療效及中藥不良反應,做主動且系統性的分析評估外,本計劃將引入西醫的轉譯醫學的研究概念進行中醫的轉譯醫學的基礎研究,以病人周邊血之免疫細胞偵測助孕酮受器種類之變化、反應作為指標,來驗證中醫辨證論治療後,對臨床與轉譯醫學預設之項目評估作為影響的證據以及與探討臨床徵狀改善的相關性。

實施方法:本研究計畫以觀察子宮內膜異位症女性經過二個月中醫辨證論治療後其症狀、生活品質、中醫證型及轉譯醫學基礎研究免疫細胞反應變化並主動與經期正常之女性為對照。

The effects of traditional Chinese medicine on women with endometriosis and the association with expressions of membrane progesterone receptors, responses and inflammation on peripheral immune cells

Background: The aim of this study was to evaluate the effects of traditional Chinese medicine (TCM) on women with endometriosis and the association with expressions of membrane progesterone receptors, responses and inflammation on peripheral immune cells Method: We will perform an observational follow-up study to examine the efficacy and safety of TCM between among women with endometriosis. We will also compare the change of expression of membrane progesterone receptors, responses and inflammation on peripheral immune cells between women with and without endometriosis.

Methods: A prospective follow-up study will be carried out in Yang Ming branch of Taipei City Hospital between January 2013 and October 2013. 20 women with endometriosis will receive traditional Chinese medicine for eight weeks. Baseline scores will be measured on pain scales, followed by further measures at the end of weeks 4, and 8. The World Health Organization Quality of Life (WHOQOL) assessment and the Menstrual Disorder Questionnaire will be undertaken as secondary outcomes. Regression models were constructed to explore the score differences between the baseline and at weeks 4 and 8 by various determinants including age, body mass index (BMI), and severity of baseline menstrual pain.

Peripheral blood immune cell from patients will be used to detect the changes on different types of progesterone receptors, cell responses as indicator to verify the improvement of clinical symptoms after treating with traditional Chinese medicine.
